All appointments are payable in full at the time of the visit. If a co-pay is an option, it should be paid in its entirety.
Any patient wishing to have insurance cover all or a portion of their treatment should contact their insurance provider to learn the coverage boundaries of their policies.

We are in network with these insurance providers:
- Blue Shield (We do not bill Blue Shield directly, however, we will issue a Superbill for reimbursement)
- Sequoia Physicians Network
- United Healthcare
We are out of network providers with:
- Blue Cross (We do not bill Blue Cross directly, however, we will issue a Superbill for reimbursement)
- California Water Service
Please note: Even though we are out of network with these providers, we can still check benefits and also bill on your behalf.
(IN NETWORK – Dr. has a contract with the insurance company)
(OUT OF NETWORK – Dr. doesn’t have a contract with the insurance company)
CO-PAY – The payment patient has to pay at the time of service
DEDUCTIBLE – The amount that patient has to pay before insurance reimburse (not every patient has a deductible that is according to their plan).
