Is it Sciatica or Piriformis Syndrome?

Sciatica or Piriformis Syndrome? Understanding the Difference

Many people use the term “sciatica” to describe any pain that starts in the buttock and travels down the leg. But what surprises many patients is that “sciatica” isn’t actually a diagnosis—it simply describes irritation of the sciatic nerve.

The two most common causes are true sciatica, where the nerve is compressed in the lower back, and piriformis syndrome, where the nerve becomes irritated by a tight muscle deep in the buttock. While the symptoms can feel remarkably similar, the cause, treatment, and recovery are often quite different.

Difference between Sciatica vs Piriformis Syndrome

True Sciatica: When the Nerve Is Being Compressed

True sciatica occurs when the sciatic nerve is irritated or compressed as it leaves the spine. This is most commonly caused by a herniated disc, spinal arthritis, or narrowing of the spaces where the nerves exit the spine. These changes are often accompanied by swelling and inflammation, which further irritates the nerve.

Because the problem begins in the lower back, patients often experience:

  • Sharp, shooting, electric pain traveling below the knee
  • Numbness or tingling in the leg, calf, foot, or toes
  • Muscle weakness
  • Pain that worsens with coughing, sneezing, or prolonged sitting

Because the nerve root itself is involved, true sciatica tends to be the more serious of the two conditions. Recovery often takes longer, and significant weakness or numbness deserves prompt medical evaluation.

Piriformis Syndrome: When the Muscle Irritates the Nerve

Piriformis syndrome produces many of the same symptoms, but the source of the problem is different.

Instead of the nerve being compressed in the spine, the sciatic nerve becomes irritated as it passes beneath (or occasionally through) the piriformis muscle deep in the buttock. The muscle becomes tight, inflamed, or goes into spasm, creating pressure on the nerve.

Patients often notice:

  • Pain deep in the buttock that is dull or achy, rarely radiates past the knee
  • Numbness is less common and usually isolated to the glutes
  • Pain after prolonged sitting or driving
  • Discomfort when climbing stairs or getting out of a chair
  • Pain that may travel down the leg, but often without significant numbness or weakness

While piriformis syndrome can be extremely uncomfortable, it generally carries a much better prognosis than true sciatica because the nerve itself is usually not being compressed at its source. Once the muscle relaxes and normal movement returns, symptoms often improve much more quickly.

How Acupuncture Can Help

When approaching the condition with Chinese medicine and acupuncture, we never assume every case of “sciatica” is the same.

Rather than treating a diagnosis alone, we listen to what the body is telling us. Tender reflexes throughout the body help identify where tension has accumulated and what structures are contributing to the pain. Sometimes we find the primary restriction in the piriformis itself. Other times, the hip flexors, abdomen, sacrum, or lower back are placing additional strain on the sciatic nerve.

By restoring smooth movement of Qi, reducing muscular guarding, improving circulation, and helping calm the nervous system, acupuncture can often reduce irritation along the nerve while supporting the body’s natural healing process.

Getting the Right Diagnosis Matters

Although piriformis syndrome and true sciatica can feel remarkably similar, they have different causes, different recovery timelines, and sometimes different treatment plans.

The good news is that both conditions often respond well to acupuncture and Chinese medicine, especially when treated early.

If you’re struggling with pain traveling from your back or buttock into your leg, our team can help determine what’s contributing to your symptoms and develop a treatment plan tailored to your body.

Whether the source is the spine, the muscles, or both, our goal is the same: relieve pain, restore movement, and help you get back to living your life comfortably.
